By proceeding with the enrollment, you agree to the following terms and conditions: Month-to-Month Subscription: The "DS On Demand" pricing plan is a month-to-month subscription that provides access to specific services as outlined in the checkmarked included items or discluded items in the listing. Setup Fee: Upon selecting the plan for the first time, you will be charged for 5 days of set up for your plan. This fee is non-refundable and covers the initial onboarding process. Auto-Renewal: Your subscription will automatically renew every month on the same date you initially set up the payment. You will be charged the applicable monthly fee unless you cancel your subscription. Onboarding Process: The onboarding process for "DS On Demand" is simplified compared to our detailed services. Onboarding will be completed within 5 days from the date of your first payment/enrollment. Business Associate Agreement (BAA): Before we can begin the onboarding process, a signed Business Associate Agreement (BAA) is required. You can find the link to the BAA in the enrollment form for ON DEMAND support. No Refunds: There are no refunds or partial refunds for any payments made towards the "DS On Demand" pricing plan. Onboarding Timeline: It is essential that your dental office completes the onboarding steps within the initial 5 days from the date of the first payment/enrollment. If the onboarding steps are not completed within this timeline, we will start the process when possible, but no refunds or adjustments to the next month's invoice will be made. Payment Due at Sign-Up: Payment for the "DS On Demand" pricing plan is due at the time of sign-up and enrollment. Cancellation: You can cancel your subscription at any time for the following month. To cancel, log into your account and complete the cancellation process. No prorated refunds will be provided for mid-month cancellations.
